Brief:
| Lamb Hospital in northwest Bangladesh has an urgent need for a short term general surgeon from now to the end of August 2010 as they currently have no surgery coverage. In addition there is an urgent need for a long term general surgeon as soon as available. If a short term surgeon could be found to cover through August, coupled with an anticipated short term surgeon coming September through December, this will temporize the coverage through the end of 2010 by which time it is hoped a long term surgeon will be have been recruited.

Lamb Hospital is 115 bed (officially, 150 capacity actual) general hospital with 24 hr. lab and X-ray services. There are full-time long-term ex-pat internists, pediatricians, and obstetricians on site. On average there are around 600 general surgery procedures done each year.
Anesthesia is primarily either local, spinal or ketemine but general anesthesia is available administered by a very competent locally trained national. Monitoring equipment is available but not including end tidal CO2.
Because of cultural issues, obstetrics and gynecology are handled by female physicians. LAMB has lady ob/gyn doctors who take care of all C-sections and other gynecological issues so surgery in these areas are not included in general surgery at LAMB if the surgeon is a male.
Apart from the above, general surgery includes all of the surgery subspecialties. Abdominal surgeries include among other things appendices, gall bladders, common duct explorations, bowel obstructions, sigmoid volvulus, ruptured liver abscesses, perforated duodenal ulcers and hernia. There is much rectal disease such as hemorrhoids, fissures, fistulas and surprisingly, a good number of rectal carcinomas in young people in their late teens and early twenties. A gastroscope and sigmoidoscope are available.
In urology the most common procedures are hydroceles, urethral dilatations, and reimplantation of ureters for women with damage to the ureters from labor and delivery complications or from hysterectomies from other hospitals gone wrong.
Orthopedics is limited because of our limitation in the necessary equipment. Common procedures include osteomylitis, some bone tumors, both major and minor amputations, and fractures when we have the necessary hardware. LAMB has an expanding club foot program for which tenotomies and tendon lengthening procedures are needed. X-rays are available but not portable x-ray and not fluoroscopy.
Other surgeries include breast biopsies and mastectomies, thyroidectomies for goiters and a few T&A's.
LAMB does not have equipment for thoracic surgery.
The post common surgeries done at present are cleft lip and palates, release of burn contractures, skin grafts and dealing with acute and chronic ulcers. There are a number of severe acute burns admitted especially during the cold season.
Emergency surgery includes primarily appendectomies and perforated duodenal ulcers but also occasional bowel obstructions, sigmoid volvulus, ruptured liver abscesses, and other small bowel perforations.
The surgery work is challenging,,also very interesting and rewarding.
A surgeon would not, of course, have to do all that is listed above. But a surgeon who could take care of the emergencies, do the routine things and who might be willing to expand into other areas would be most welcome.

Lamb Hospital has a new training facility with 25 rooms, and conference facilities. Staff accommodation is available on site. The project has over 600 staff and 400 community volunteers. Travel from Dhaka is by plane, train or 7 hours by car. |
